Here at ParkWest Surgery Center we aspire to provide reliable information to help patients understand pricing in advance of their surgical procedure. For our self-pay or uninsured individuals, we offer a Good Faith Estimate of the expected pricing for their procedure.  The pricing information provided to our self-pay or uninsured patients is considered an estimate for the procedure they have scheduled based off of the common procedural terminology codes given by their surgeon office.  The pricing covers only the stated service scheduled as listed and does not include fees for services such as those provided by a laboratory, surgeon or anesthesiologist. Please contact the surgeon and anesthesiologist at their office directly for price information associated with their care and services.

Please note: If your surgeon indicates you will be receiving implants during your procedure, these will be in additional to the prices listed below. You will be expected to pay for your implants when you provide payment for your surgery. In the event that all of the implants are not utilized, a refund will be given.

 

This pricing does not apply to patients who have health insurance coverage through Medicare, Medicaid, other government insurance programs, or a commercial insurance company. If a patient has health insurance, the patient’s health insurance policy (including deductibles, co-pays, coinsurance and out-of-pocket maximums) will apply and the amount the patient owes will depend on the patient’s insurance coverage. Pricing noted below for health care services may not reflect the actual amount of your financial responsibility.  The pricing information is not a guarantee of insurance coverage or availability of services.
